The reason why he does crazy stuff when looking up, is accorrding to me because he doesn't have enough stability in his take off. It's the same when you stand on 1 foot and look up. It will be much harder to ballance than when you look down.
Looking down also makes you stay on the bottum too long.

You have a sollid take off, but personnally I think it is a bit too sollid. meaning you drive the knee, but too long. that way you're on the bottom too long and you're missing valuable time at the end. the pole start unbending to soon for you (hips are not high enough) and so your feet drop. with a ...

After reading the 6.40model you have posted, I have to agree with you on that. it is indeed the visual angle. and I guess it depends from vaulter to vaulter. It is importent to search active your highest point with your top arm. this without loosing speed.
