Keep your hips in line with your shoulders on take off, by shortening your last step to leave the ground in a very slight forward angle. From there, your trail leg should come behind you naturally, and you will have a much longer axis to swing on.

See a PT. There is no "one" exercise that will fix you or make you good at pole vaulting. Especially if you are rehabilitating your shoulder, you are going to want to be doing several rotator cuff exercises, back exercises, and shoulder/neck exercises with low weight, probably sets of 10-1...

Just something to add to Scott's comments, your position on take-off is decent but your hips start moving in front of your shoulders before your trail leg has fully extended. You have a tendency to pull after you leave the ground. Try to stretch your hands high and "hang" from the pole after a strong take off. Keep your trail leg straight as you swing it through. Also, you are using your bottom arm to help accelerate your swing. You don't have to be pulling wit...

Try the glue or at least practice with some tape to hold it in place till the meet.
Also make sure you are letting it dry up at night. Wipe it with alcohol, it hurts like hell but it drys the area up nice, and make sure you are letting it air out at night.

Sounds Good!
As long as it isn't over-used or too tall I think it can be a good tool. Just make sure you avoid getting too comfortable with it, and avoiding knee injury by keeping the runs short and the box short.
