Who: All Pole-Vaulters young and old are invited!

Cost: $25(includes $10.00 non-refundable application fee) for all entries post marked no later than August 1st, 2009(late fee is $10).

Awards for all division winners.

The Score Restaurant and Sports Bar is Grand Rapids premier location for Sports related food and fun. With fantastic food, over 50 HD TV’s, pool tables, basketball hoops, darts, games, and state of the art sound system, it is the place to be!

The Pole Vault location is just north of The Score, located on a nice grassy area, with a little shade from the trees that overlook a refreshing lake. The dining deck(located next to the competition area) will be open for dining pleasure!

I verify that my child has had a physical exam in the past year and is capable to participate in the activities related to the clinic. I realize that pole vaulting is a potentially dangerous activity that could result in serious injury, paralysis, and even death. I agree to indemnify, hold harmless and defend Dave Emeott, Kevin Patterson, KP Athletics,LLC, volunteers, Plainfield Township, The Score Restaurant and Sports Bar and/or their employees from any liability and negligence for injury to my child, as well as any injury caused by my child. . I also hereby grant permission to KP Athletics, LLC to use any photography and videotape of myself and/or child related to Score Vault activities for advertising, promotional, sales, or educational video materials.
