They are, but the other person has not read it yet. This happens a lot if you are trying to PM someone whose email address is no longer valid.

Is that why they seem to stay in the Outbox?

Yes. If they are in the outbox it means the other person has not read it. Unfortunately many of you are bad about keeping your email address with PVP current. So the email notification that you have a PM goes to a bad email address and many people either never notice or they don't log in to the board often enough.
